IDENTIFICATION REQUIREMENTS

Administrative
DEFINITION
All applicants for drivers' licenses or identification cards are required to present proof of identification as defined herein.
FIRST TIME APPLICANTS
Applicants will be required to provide one of the following combinations:
- One primary and two secondary documents OR
- Two primary documents OR
- If applicant has a Louisiana identification card/driver's license with a photo which clearly identifies the applicant additional identification is not required.
ADDITIONAL REQUIREMENTS FOR APPLICANTS SEVENTEEN AND UNDER
- The signature of the custodial parent or legal guardian is required for the issuance of any license (including first time applications, duplicates and changing restrictions).
- Documentation must be presented to prove custody or legal guardianship.
- Identification must be presented by the minor and the parent or guardian.
- Only the domiciliary parent may sign if joint custody has been awarded.
- A completed Certification of Required Attendance form (effective September 16, 2013).
SOCIAL SECURITY REQUIREMENT
- All applicants eligible for a social security number must provide the assigned number when applying for a driver's license. Although the Social Security Number will not be displayed on the face of the license, that number must be entered in the department's internal records.
- Any alien residing in Louisiana who does not possess and is ineligible to obtain a social security number shall be required to furnish a letter from the Social Security Administration stipulating they are "ineligible" or that they "have not been issued" a social security number for a Class D or E driver's license.
- In order to comply with the Military Selective Service Act, R.S. 32:40:1321(D)(1), the Social Security Number is required from all male US citizens or immigrants age 15 to 26 who apply for a driver's license. Failure to provide the SSN will result in the denial of issuance.
OUT-OF-STATE TRANSFERS
Applicants must provide all of the following:
- Out-of-state license
- One primary document
- One secondary document
- Social Security Number
EXCEPTIONS ON OUT-OF-STATE TRANSFERS
- If the applicant has had a previous Louisiana license or identification card and it is currently on photo retrieval and the applicant is clearly identified, then the above requirements may be waived.
- If the out-of-state photo license has been lost, an official driving record from the last issuance state must be provided.
DUPLICATES
When an applicant eighteen or over applies for a duplicate Louisiana driver's license or a duplicate identification card and a photo image does not exist under the Photo Retrieve Program for verification purposes, the applicant will be required to provide one of the following combinations of identification:
- One primary and one secondary document, OR
- Three secondary documents, OR
- A Louisiana driver's license or a Louisiana identification card, provided the photo clearly identifies applicant.
ACCEPTABLE PRIMARY & SECONDARY IDENTIFICATION DOCUMENTS.
PRIMARY DOCUMENTS
Birth Certificate
- Original certificate of birth
- Certified copy of birth certificate (long form) and Birth Card (short form) does not require a seal
- Birth registration card or certificate issued by a state or county bureau of vital statistic
- Certificate of Birth Abroad issued by the US Department of State.
- Certificate of Naturalization
- Native American tribal document
Passport
- US Passport book or Passport Card
○Applicants 17 and under must also present a certified birth certificate or documentation proving custody/legal guardianship
○Must be appropriately stamped and accompanied by proper immigration documents
Immigrants / non-immigrants
- Permanent Resident Card (I-551)

SECONDARY DOCUMENTS
Out-of-state driver's license / identification card
- DL or ID card issued by a state motor vehicle department with a photo that clearly identifies the individual.
- An official driving record from the last state of issuance is required if the applicant has lost his out-of-state license.
- A foreign driver's license must be accompanied by proper immigration documents.
Social Security Number verification
- Social security card
- Official verification of the social security number from the Social Security Administration or verbal verification
Identification Card (with photo)
- Louisiana college or university
- Louisiana middle / high school
- Law enforcement officer's ID
- Employment ID card (major corporations, hospitals, governmental agencies)
- ID cards issued by federal, state, local governmental agencies
- LA Department of Public Safety & Corrections prison or parish ID card
- TWIC (Transportation Worker Identification Credential)
- Employment Authorization Document I-766 issued by DHS
- Original Refugee Resettlement Program letter. This document will include the agency director's signature with a raised seal and is valid for 15 days from issuance.

- Motor vehicle lien instrument
- Local utility statements showing name and address of the applicant or a receipt indicating utilities have been turned on

- Prison release documents or letter from probation officer
- Letter of verification / introduction from another state agency responsible for placement of deprived / impaired individuals (i.e., Blind Services)

ELDERLY AND DISABLED APPLICANTS
Every effort should be made to accommodate the elderly and disabled.
EXCEPTIONS ON OUT-OF-STATE TRANSFERS
- If the applicant has had a previous Louisiana license or identification card and it is currently on photo retrieval and the applicant is clearly identified, then the above requirements may be waived.
SPECIAL NOTES
- If the applicant has a Louisiana driver's license and applies for an ID card, or if the applicant has a Louisiana ID card and applies for a license, the name and address on both credentials must match.
- Photocopies of documents will not be accepted.
- Law enforcement officers, court officers, and members of their immediate families as defined in R.S. 32:409(C) shall be given the option of providing their official work address or post office box address instead of a residential address. If a person exercises this option, then the person shall not be eligible for voter registration at the driver's license facility as provided for in R.S. 18:114. DPSMV 1624 form must be completed and submitted with original proof of employment or retirement. The proof of employment or retirement must be dated within the most recent 6 months. The original form and original proof of employment or retirement must be submitted along with application for imaging.
DUPLICATES
- If a photo image does exist under the Photo Retrieve Program and the applicant’s identity can be verified, the photo image will be considered sufficient identification (no secondary documents are required).
- If more than one retrieval image is present, all photos on retrieve must be reviewed. If the photo does not clearly identify the applicant or if the photo does not match, contact your supervisor.
ISSUANCE OF P-2
If an individual is applying for a duplicate driver's license and cannot provide the identification documents above, and if the Motor Vehicle Compliance Analyst feels comfortable that the individual is who he claims to be, the officer may exercise his discretion by issuing a P-2 for 60 days or less. This will allow that person to drive and give him time to acquire the proper documentation
PRIMARY AND SECONDARY DOCUMENTS
- The names on the primary and secondary documents must match.
- Do not accept any document that is not listed without approval from your office manager or supervisor.
NOTES:
- I-551: If the photograph was taken prior to the applicant's 14th birthday and he is now over age 14, he must be referred to the Department of Homeland Security for issuance of a new I-551 prior to processing.
- N-550: This document grants U.S. Citizenship.
SCHOOL ENROLLMENT FORM
Effective September 16, 2013, all minor applicants will be required to obtain a complete Certification of Required Attendance form from his school office prior to applying for a driver's license. The form was developed by the LA Department of Education and has been disseminated to all secondary high schools. The form provided must be in the acceptable format (a sample of which is attached) and must be completed in its entirety. One block must be checked for #4 on the form, which indicates the applicant's enrollment status
- A current Certificate of Required School Attendance must be submitted or verified each time a minor makes application with the Office of Motor Vehicles (to lift the 02 restriction, be issued a duplicate, be issued a Class E (at age 17) or request a change of address). Effective 08/01/2014, the current school attendance form is valid for 90 days from the date of issuance.
- For home study or registered nonpublic school students, the parent/guardian must submit one of the following documents each time a minor makes application with the OMV (as stated above) :
- SBESE home study - signed and approved Home Study Approval Notification Letter. This letter is valid for one year from the date on the letter and must be renewed annually.
- Private option - signed Registered Nonpublic School: Registration Confirmation Letter verifying registration of the nonpublic school. This letter is valid for one year from the date of the letter and must be renewed annually.
- If the applicant has graduated or attained the high school equivalency diploma, he may present the diploma upon application in lieu of the Certification of Required Attendance.
