After April 30, all truck renewals, new registrations as well as the inspection must be performed through a Commission Inspector. ALSO, AFTER APRIL 30, NO INSPECTIONS, RENEWALS OR NEW REGISTRATIONS WILL BE HANDLED BY MAIL. Upon receipt of the transmittal with the tag attached the inspection must be performed within 90 days of the “Inspection Due By” date or by August 1, 2000 whichever comes first. 3. Did allow driver, Egbert N. Grace, to operate truck #770, trailer #10372 over the highways of the State of Louisiana without the vapor outlet properly labeled as required; thus violating LAC 55:IX.181 (A), NFPA 58 - 1995 6-3.6. Found guilty and assessed a penalty of $150.00.

1. Failed to affix the dealer name on a storage tank site located at the residence of Sherman Ashworth, Oakdale, LA as required; thus violating LAC 55:IX.113 (1)(f). Postponed until the March meeting.

FIRES AND ACCIDENTS

Several dealers reported four related fires damaged a home, destroyed two mobile homes and a minor fire was reported at a dealers office. The fires are believed to have been started two from heaters being turned on, a water heater and possible static electricity. There was loss of product and injuries were reported.

Several dealers reported eight unrelated fires and five unrelated accidents. The fires destroyed two homes and a mobile home and damaged a chicken house. The fires are believed to be caused from electrical problems, arson and the others are unknown. There was no loss of product and no injuries were reported.

Several dealers reported five accidents occurred with their vehicles. In one accident the driver reach to pick up a coke he had dropped and he lost control of the vehicle, another the vehicle ran off of the road, another accident their vehicle was struck head on, and last when a dog bit into a hose on a propane tank. There was loss of product and injuries were reported.
